Pyrimethamine has an inhibitory effect on the infrared phase of certain falciparum malaria and Plasmodium vivax, and its inhibitory effect on the red inner phase is limited to the immature schizont stage, which can inhibit the division of trophozoites. Plasmodium cannot use the folic acid that appears in the environment, but must synthesize it by itself. Pyrimethamine is an inhibitor of dihydrofolate reductase, which prevents dihydrofolate from being reduced to tetrahydrofolate, thereby affecting purines and pyrimidine nucleotides. The biosynthesis of serotonin, finally reduces the synthesis of nucleic acid, which inhibits the division of cell nuclei and the reproduction of Plasmodium. The DNA synthesis of Plasmodium mainly occurs in the trophozoite stage, and little synthesis occurs in the schizont stage. Therefore, pyrimethamine mainly acts on the schizont proliferation of the plasmodium, but is not effective on the developed schizont.
