Topiramate is a monosaccharide substituted with sulfamate. The exact mechanism of topiramate against epilepsy is not clear. In the electrophysiological and biochemical studies of neuronal cells cultured in vitro, it was found that three properties of topiramate may contribute to its anti-epileptic effect. Topiramate can block repetitive action potentials induced by the continuous depolarization of neurons. This effect is time-dependent, indicating that topiramate can block sodium channels in a state-dependent manner. Topiramate can increase the frequency of γ-aminobutyric acid (GABA) activating GABAA receptors and strengthen the influx of chloride ions into neurons induced by GABA, indicating that topiramate can enhance the effect of this inhibitory central neurotransmitter.
