Aldesonium bromide is a long-acting inhaled anticholinergic drug developed by American Forest Laboratories Pharmaceuticals and Almirall. It was approved by the FDA in 2012 for the long-term maintenance treatment of chronic obstructive pulmonary disease (COPD)-related bronchospasm. The mechanism of action of aclidinium bromide is to promote bronchiectasis by inhibiting the effect of acetylcholine on tracheal smooth muscle muscarinic receptors. Alclidium bromide is the third anticholinergic bronchodilator to be marketed after ipratropium bromide and tiotropium bromide. Its onset speed is faster than tiotropium bromide. It is highly selective for M3 cholinergic receptors, and can bind to M2 and M3 receptors after entering the body. The half-life of M3 receptors is 6 times that of M2 receptors. It is a long-acting cholinergic receptor antagonist. Its adverse effects against M2 receptors, such as tachycardia, are relatively rare. In addition, since aclidinium bromide quaternizes the tertiary amino group, it reduces its oral bioavailability and ability to penetrate the blood-brain barrier. After inhaled administration, there are fewer systemic adverse reactions. Some studies believe that aclidinium bromide is very effective in the treatment of COPD-related bronchospasm and COPD-asthma overlap syndrome, and it is recommended that aclidinium bromide be used as the first-line medication for COPD patients. In 2018, the scale of domestic asthma and COPD medications in China was 4.215 billion yuan, a year-on-year increase of 9.57%. Aclidinium bromide is a respiratory product among the top 500 best-selling drugs in the world, with sales of 110 million US dollars in 2018.
