As a typical pyrimidine antimetabolites, cytarabine is one of the most commonly used chemotherapeutics in clinical practice, and it is also the first chemotherapeutic drug that changes the effect of nucleoside itself. Cytarabine is very similar to deoxycytidine, so that it can be incorporated into human DNA instead of the latter. However, the structural difference makes DNA unable to replicate and kills affected cells. The latest research points out that low-dose cytarabine combined with decitabine in the treatment of senile myeloid leukemia is beneficial to promote the recovery of cellular immune function, and is the first choice for high safety, low adverse reactions and good efficacy. Cytarabine is currently one of the most effective drugs for the treatment of acute non-lymphoid myeloid leukemia in my country. In November 2018, the U.S. FDA approved Pfizer’s Hedgehog signaling pathway key receptor SMO antagonist Grajib and low dose Cytarabine (LDAC) is used in combination as the first-line therapy for AML patients over 75 years of age or unable to use high-intensity chemotherapy due to complications. For some specific chemotherapy regimens, the combination of chemotherapy drugs and cytarabine can achieve a cure rate of 40% to 50% for leukemia, but if there is no cytarabine involved in the treatment, the cure rate is 0%. For these patients, the difference between using cytarabine is life and death.
