Mitiglinide calcium is a derivative of phenylalanine. It is a type 2 diabetes treatment drug developed by Japan's Tachibana Pharmaceutical Company. The original product was approved for marketing in China in 2010 and has the reputation of "in vitro pancreas". Mitiglinide calcium closes the ATP-dependent K channel on the pancreatic β-cell membrane, causing Ca influx, increasing the intracellular Ca concentration and degranulating the extracellular insulin-containing vesicles, thereby stimulating the secretion of insulin. Traditional oral diabetes drugs continue to stimulate insulin secretion. When using sulfonylurea drugs, patients should even be encouraged to take snacks between meals. Mitiglinide calcium only stimulates pancreatic secretion quickly and briefly during meals. Insulin has fast onset and short duration of action. Researchers compared the effects of mitiglinide, nateglinide and voglibose on lowering blood sugar after meals, and found that mitiglinide has the most significant hypoglycemic effect. At present, the market size of Mitiglinide calcium is still small, but its mechanism of action is novel and faster onset. It is a first-line treatment for patients with early and mild diabetes. It will gradually replace sulfonylurea drugs and has a large market potential.
